Job Description
Qualifications:
Experience working with the persistently mentally ill and/or co-occurring disorder adult population in a Community Support Program.
- Master’s or Bachelor’s degree in mental health/co-occurring services or related field from an accredited college/university.
- Bilingual in Spanish preferred but not required
- Case manage and advocate for caseload of adult consumers with co-occurring disorders
- Preferably 3000 hours experience working with the population.
- Effectively contributes as a multi-interdisciplinary treatment team member/leader
- Knowledge of the Assertive Community Treatment (ACT) and Integrated Dual Diagnosis Treatment (IDDT) model of care
- Ability to provide individual and group therapy
- Ability to coordinate the services of community agencies providing social, health and rehabilitation services for the target population
- Document client’s problems and formulate individual recovery plans to meet those needs
- To make home visits, arrange for physical, medical and/or psychological assessments and/or treatment, including alcohol and other drug abuse (AODA)
- Provide on-call services during the week, weekend and holidays on a rotated schedule and make necessary contact as dictated by the precipitating crisis involving the client
- To attend staff development programs and in-service training
- Must have a reliable insured vehicle
