Full Job Description
Under minimal supervision, perform reviews of chart patient charts via remote EMR access and record findings in a proprietary software application. Attend team clinical meetings focused on education and calibration.
ModusOne Health is actively seeking a team member to perform patient chart reviews and record clinical findings. This is a remote position with flexible work hours, and specific training is included to perform the job duties. Part-time to start with potential for increased hours.
The Chart Reviewer role performs ongoing analysis of charts for the appropriate review of specified conditions for compliance in clinical documentation accuracy, reflecting accurate representation of the severity of illness, expected risk of mortality, and complexity of care.
Job Duties:
- Review clinical documentation records to facilitate the accurate representation of illness and complexity of care with respect to a list of targeted conditions
- Meet and maintain production and quality expectations for all assigned measures.
- Perform daily responsibilities with acute attention to detail.
- Uses Independent judgment to complete abstraction and review of medical records following program guidelines.
- Responsible for managing and utilizing confidential information from medical records in an appropriate manner following designated guidelines including HIPAA and company policies/guidelines.
Requirements:
Medical chart reviewing, medical records reviewing, or chart abstraction experience
High speed internet with good service available at your home or work space.
Ability to work in a web-based application
Registered Nurse or equivalent certification
Ability to use Microsoft applications such as Outlook
Knowledge of HEDIS concepts and principles.

Compensation will be based upon years of experience, education, and credentials.
