Orion Family Services, an innovative community mental health agency, is looking to add a dedicated Service Facilitator to our team. If you are passionate about making a difference in the lives of others, you will find a home here at Orion. We are a licensed outpatient mental health clinic located in Madison, Wisconsin, which provides an array of community services throughout southern Wisconsin. Our Service Facilitators work directly with children and their families in Dane County who have been enrolled in the Dane County Comprehensive Community Services (CCS) program. They focus on providing case management services that aid in psychosocial rehabilitation such as completing comprehensive assessments, overseeing recovery planning, coordinating referrals for services, identifying and providing resources, facilitating multidisciplinary team meetings, etc. We value the provision of meaningful and compassionate services dedicated to healing the minds and spirits of those we serve. Qualifications:
- Bachelor’s or Master’s Degree in a social work, human services, or related field
- Experience providing case management services to children and their families with mental health needs
- Experience working within a multidisciplinary team
- Must pass WI Caregiver background check
- Valid driver’s license, insurance, and good driving record
- Must pass required background checks
- Professional written and verbal communication skills
- Commitment, Integrity, and an ability to laugh easily!
