TITLE: Mental Health Practitioner – PAIT

DEPARTMENT SUMMARY: The Psychiatric Assessment and Intake Team provides services to the community and region including but not limited to: acute mental health crisis assessment requiring intervention and/or hospitalization; intake coordination for acute mental health patients being referred from health systems outside St Charles Health and collaboration with outpatient and community providers at discharge. The Inpatient Behavioral Health Department provides services to the community and region including: treatment for the SPMI population, acute mental health crisis requiring hospitalization, and collaboration with the outpatient community.
POSITION OVERVIEW: The Mental Health Practitioner provides mental health services including: crisis assessment; psychosocial evaluation; diagnosis; brief treatment interventions for clients/patients experiencing social/behavioral/mental health issues; discharge planning; warm hand-off and follow-up caring contacts. This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Participate with team members in facilitating patient’s and family’s learning throughout the continuum of care. Reinforce patient’s continued health care through teaching and/or referral to community agency follow-up.
Perform patient assessment of social, economic, emotional and family situations.
Provide diagnostic, preventative and treatment services of a psychosocial nature pertaining to personality adjustment, behavior problems and interpersonal dysfunction.
Respect the privacy and protect the confidentiality of patients and family members while maintaining the boundaries of the professional discipline.
Function as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost-effective manner.
Work in a fast past environment at times multi tasking and triaging numerous needs of patients, their families and community agencies.
Complete Psychosocial Assessment, trauma assessment, substance abuse assessment; suicide safety plan and occasionally a treatment plan that is individualized and measurable in compliance with the department and regulatory standards.
Assess the potential threat of harm to self and/or others and/or inability to care for one self. Determine, in coordination with the treatment team and county, the legal status of patients in accordance with state Civil Commitment laws.
Work in concert with the psychiatrist to admit patients to acute psychiatric inpatient treatment by developing a working diagnosis and treatment goals.
Provide brief individual and family therapy using solution focused treatment modalities, interventions and strategies. Provide psycho-education to patients and their families. Provide thorough documentation of all patient interactions in accordance with hospital, state, federal and regulatory requirements.
Coordinate discharge planning with outpatient providers including but not limited to therapeutic appointments, medication management and follow up medical appointments.
Provide crisis intervention to patients in immediate crisis in the emergency department/hospital medical unit(s) and to recently discharged patients on an as-needed basis.
Participate in AVADE interventions as needed.
Responsible for maintaining knowledge of all state and federal regulations as they apply to the admission, care, and discharge of patients from an emergency department/hospital medical unit including all pertinent civil commitment procedures and protocols.

Occasional travel between Bend, Redmond, Prineville and Madras hospitals may be required.

EDUCATION
Required: Master’s degree in the field of Mental Health: Social Work, Counseling, Psychology or related
Preferred: N/A
LICENSURE/CERTIFICATION/REGISTRATION
Required: Valid Oregon driver’s license and ability to meet SCHS driving requirements.
Preferred: Professional license or in the process of getting licensed. Member of the National Association of Social Worker’s, American Counseling Association or another national association membership.
EXPERIENCE
Required: One year of experience in mental health field (internship experience in an emergency department/inpatient behavioral health setting may be considered)
Preferred: Experience in an emergency department/inpatient setting. Post graduate, clinical experience
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
