CSI Companies is looking for a Certified Medical Assistant/Care Coordinator in the San Francisco area. This is a direct hire position on a REMOTE schedule.

Title: Certified Medical Assistant/Care Coordinator
Pay: $20-$23.75/HR, Depending on experience
Hours: Full time, M-F 8 to 5. Remote Schedule
Location: Remote. Must live within 50 miles of San Francisco, CA
Responsibilities
- Enrolling patients in the Circle of Care Program
- Educate patients on the benefit of the program
- Assist patients in creating realistic goals and oversee their progress.
- Create, review, and revise patient’s plan of care
- Accurate documentation
- Help customize care plans and guidelines for managing chronic conditions
- Act as an advocate and information resource for patients
- Establishing a trusting, supportive, and collaborative relationship with patients and their families.
- Daily review to ensure new and established patients are scheduled for follow-up appointments.

- Ability to communicate effectively both verbally and in writing

Education and Experiences
- Completion of an accredited Medical Assistant program
- At least 2 years of relevant experience
- Proficient in Internet Navigation, Microsoft Word, Outlook, and Teams, EHR/EMR, and AthenaNet preferred
- Nephrology Experience or Dialysis experience Preferred
- Comply with HIPAA, OSHA, Compliance, and IT standards and policies.
