Western NC Community Health Services (WNCCHS), a non-profit, Federally Qualified Community Health Center (FQHC) with 15 primary care and 9 integrated behavioral health providers, seeks a Medical Director of Population Health. The Medical Director will provide direct patient care 3.5 days per week with 1.5 days administrative time. He/She provides clinical and strategic leadership and direction to all population health initiatives. The Medical Director will develop and execute strategies to improve patient care and meet the clinical objectives of all population health and risk-sharing arrangements. Collaborates with operational and finance leaders across the organization to enhance the delivery of care, adapt “best practices,” and develops and implements value-based financial incentive models, including upside gain-sharing arrangements, quality improvement incentive programs, governmental incentive programs, and other third party pay-for-performance programs. Population Health initiatives include but are not limited to quality improvement, risk score accuracy, government programs, KPIs, and improvement in ineffective utilization. The Medical Director helps clinicians focus on models of care that advance the health of the community and the individual, control cost, and enhances reimbursement through value-based purchasing and shared risk opportunities. The Medical Director will need working knowledge of P4P programs and care management models (ACOs, medical homes). Experience and demonstrated successful leadership of case management, disease management and /or population health programs. Knowledge of the concept of risk and how the application of risk management theory can alter population health outcomes. Experience working with the application of data analytics and performance measurement tools for improving patient outcomes.1. In partnership with the CMO and Director, Population Health, identifies opportunities to maximize performance on cost, quality, patient experience and clinical integration.2. Drives the strategy, design and implementation of population health initiatives related to quality improvement, risk score accuracy, government programs and KPIs.3. Works to translate integrated care management vision into strategic and tactical plans by considering innovative and strategic differentiators within the market and assessing the ongoing requirements for population health management.4. Serves as a key player in the delivery of culture change and provider engagement including adoption of value-based and patient-centered principles and related care delivery innovations.5. Implements programs and processes to improve the overall health of patients by developing, monitoring and refining wellness and disease management strategies, programs and initiatives.6. Creates an environment that fosters provider and staff collaboration to enhance revenues, control costs, manage utilization, develop new programs and continuously improve clinical quality, customer service and overall performance.7. Communicate objectives of the population health program with clinical, operational and financial stakeholders.8. Leads educational efforts to help providers understand the population health program and functions and orients providers to the program, concepts and processes.9. Provides up to 0.7 FTE of direct medical care within WNCCHS to serve high- risk patients with complex social, environmental, and mental health needs.10. Identifies opportunities through data analysis for improvement in the medical group that will drive the success under Population Health ProgramsMinimum of 5 years in a clinical administrative role within a primary care setting. At least 2 years’ experience with governance and management in an established ACO and/or directing value-based reimbursement models.
