Care Manager/CM,Full time, Day Shift, Kenosha, Wisconsin
Community Care is hiring a Care Manager to join our Family Care Program team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule. It is based at our Kenosha facility (currently telecommute is more the norm).
Care Manager Job Responsibilities:

Manages an assigned caseload of program members in collaboration with an RN Care Manager
*
Completes comprehensive member assessments, member centered care plans, & functional screens
*
Provides ongoing care coordination and quality assurance monitoring of services
*
Maintains required documentation in adherence to established standards.
Care Manager Job Requirements:
Education:Master’s degree in Social Work, Psychology, Human Services, or closely related field.
*A Bachelor’s degree in Social Work, Psychology, or Human Service and 18 months of the experience specified below also qualifies.
Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in
a social service capacity.
Certification: Eligibility for long-term care functional screener certification is required
Travel: Regular local travel, requiring use of personal vehicle, valid driver’s license, and proof of required insurance coverage.
