Statement of Purpose: While living well with an HIV+ diagnosis, able to help others navigate the challenges of living with HIV. Create rapport, share experiences and access clients’ needs. Assist with client education to improve their adherence to medical protocol, their coping skills, and quality of life. Liaise with the Medical Teams, Case Management, Patient Care Coordinators, etc.

Primary Tasks: Medical Peer Navigator

• Must be comfortable disclosing HIV status to clients and relevant staff.
• Implementation of the core values of the Peer Navigator program.
• Assess and address clients’ knowledge, attitudes and beliefs regarding HIV.
• Assess clients’ adherence, willingness, and abilities regarding antiretroviral therapy.
• Assess clients’ needs and create referrals for services.
• Assist clients with strategies to overcoming barriers to care.
• Educate clients about support groups, educational opportunities, etc.
• Address a prevention care plan to improve overall health.
• Review case notes, labs, and medical encounters to coordinate care.
• Provide HIV education and sexual risk reduction counseling regarding HIV and sexually transmitted infections.
• Documentation of all client interactions and activities in EMR system.
• Communicate with medical team, case management, etc. through Qliq email.
• Plan, attend, and facilitate Peer related groups and events.
• Ability to work with a multidisciplinary team.
• Educate clients about the 340B Program and their pharmacy choices.
• Participate in QAQI, Interdisciplinary Care, and other relevant meetings.
• Maintain knowledge of medical terminology and practices pertinent to HIV care.
• Follow-up with no-shows and contact clients with appointment reminders.
• Submit reports in a timely fashion.
• Promotes and practices CAN Community Health, Inc.’s mission and values, and follows its policies and procedures.

Secondary Tasks: Medical Peer Navigator

• Ensure confidentiality is maintained regarding patient/client information in accordance with HIPAA.
• Participates in continuous quality improvement of services rendered to CAN Community Health clients.
• Develops and fosters networking relationships with community and support resources.
• May accompany client to appointments outside of CAN.
• Participation in the HIV Speakers Bureau is encouraged.
• Stay up-to-date with the new data/science in the field of HIV.

Requirements:

Knowledge, Skills and Abilities Required: Medical Peer Navigator

• Adept at navigating cultural differences.
• Adept with working with persons of color, gender and the lesbian/gay/bisexual/transgender/queer community.
• Understanding on how HIV is transmitted and prevented as well as familiarity with antiretroviral therapy.
• Ability to work independently with accountability.
• Exercise sound judgment, discretion and professionalism.
• Sound communication and interpersonal skills with patients, visitors, fellow staff members and the supervisory team.
• Bi-lingual is preferred.
• Familiar with basic computer skills and use of social media.
• Working knowledge of Microsoft Office products.
• Demonstrated oral and written communication, organizational, multi-tasking and problem-solving skills, ability to work under pressure to meet deadlines.
• Able to work with minimal direct supervision.
• Ethical conduct.

Physical Demands: Medical Peer Navigator

• Neat, professional appearance.
• LIGHT- Exerts up to 20 lbs. of force occasionally, and/or up to 10 lbs. of force frequently, and/or a negligible amount of force constantly to lift, carry, push, pull, or otherwise move objects, including the human body. Involves sitting most of the time, but may involve walking or standing for brief periods of time.
• Requires expressing or exchanging ideas by means of spoken word, visual and auditory acuity.
• Valid driver’s license and ability to operate a motor vehicle.
• Travel between facilities and outside agencies.

Education/Professional: Medical Peer Navigator

• Experience as an outreach worker or case manager is preferred.
• Relevant HIV experience is required.
