The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions – the San Francisco Health Network and Population Health. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.
Positions in class 2586 Health Worker 2 may be located at the following locations, but not limited to: Zuckerberg San Francisco General Hospital (ZSFG), Laguna Honda Hospital (LHH), various clinics, and other City Departments such as the Human Services Agency (HSA), the Department of Homelessness and Supporting Housing (HOM), Community College District (CCD), and the Unified School District (USD).
Shift: Days, evenings, weekends, and/or holiday shifts may be required, depending on the position.
The eligible list resulting from this recruitment may be utilized to fill future and current vacancies in this class at other locations within the DPH for the duration of the eligible list. 
Job Description
Under supervision, the Health Worker 2 performs a wide variety of paraprofessional duties in a service program, and functions as a liaison between community residents and program staff; provides counseling and advice to patients regarding health problems; may supervise Health Worker I, and may drive or accompany patients between their homes, hospitals or other social agencies. The essential functions include, but are not limited to the following:
The 2586 Health Worker 2 may perform other related duties as required/assigned.
Working Conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting.
Qualifications
Experience: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:
Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification #1.
One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.)

Minimum Qualification Supplemental Questionnaire (MQSQ):
Candidates will be required to complete an MQSQ as part of the employment application. The MQSQ will be used to evaluate if the applicant possesses the required minimum qualifications.
Assessment Component
Multiple-Choice Exam (Weight: 100%)
Candidates that meet the minimum qualifications will be invited to participate in a multiple choice exam that is designed to measure the knowledge, skills, and abilities in job related areas which may include but not be limited to: Ability to establish and maintain effective and cooperative working relationships by demonstrating leadership; Ability to work in a culturally diverse setting with clients who are disabled, physically and chronically ill and/or who may have behavioral health issues; Ability to apply de-escalation and counseling techniques when dealing with patients/clients; Ability to interview patients/clients to evaluate their capabilities, needs, interests and the ability to exercise good judgment when making a patient/client determination; Ability to read, comprehend and interpret information to communicate in a clear and concise manner including the ability to provide educational information about health programs to patients/clients; Ability to type and enter data/information accurately and the ability to coordinate and schedule patient/client appointments; Ability to prepare, organize, code and index records and files; Knowledge of and ability to interpret and apply office policies, laws, regulations (e.g. HIPPA) and procedures in a medical setting.
Candidates must achieve a passing score in order to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final exam score.
This is a standardized examination; therefore, test questions and test answers are not available for public inspection or review.

Language Proficiency Exam: For positions requiring bilingual fluency, candidates deemed qualified who meet the special language condition must pass a departmentally approved language proficiency exam prior to appointment. Only those eligible candidates who pass the bilingual proficiency test will be considered for the bilingual position. Applicants must indicate on the application form the language(s) in which they claim proficiency.

One-year fulltime employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

The certification rule for the eligible list resulting from this examination will be the Rule of 10 Scores.

The duration of the eligible list resulting from this examination process will be 12 months and may be extended with the approval of the Human Resources Director.
