HomeCare & Hospice of the Valley is actively searching for experienced, well organized, motivated Home Health nurses.
$4,000 reimbursement relocation costs or sign-on bonus. $2,000 upon hire and completion of orientation and full case load assumed; the remainder paid after successful completion of 1 year of employment (not available to part-time applicants)

Area of Service: Vail/Eagle Valley
Pay Range: $5,500 to $8,320/month
Essential Functions of the Job:
- The Home Health Nurse performs initial assessment upon admission to Home Health
- Admits patients to Home Health service by accurate entry of patient information, admitting diagnosis and physician’s orders
- Develops and implements an individualized, realistic, goal and outcome-focused Plan of Care for each patient and updates the Plan of Care per department guidelines. Plan of care relates to admitting diagnosis and/or to changing needs of patient
- Case manages patient care when assigned to perform Case Management Duties
- Home Health Nurses must participate in Call rotation with one call shift per week and one weekend of call per month

Requirements:
Qualifiications for our Home Health Nurse
- The Home Health Nurse must possesses current license to practice as a registered professional nurse in the state of Colorado.
- Current BLS verification.
- Minimum one year of Home Care nursing experience
- One year of acute care experience
- Personal vehicle for on-the-job transportation in all road and weather conditions, climate
