Description
The Case Manager,NUR CASE MGR1 reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
Reports to the Manager of Case Management. The Case Manager participates in the coordination of care and services across the continuum to facilitate the achievement of cost and quality of patient outcomes through communication, coordination and collaboration. Coordinates effective utilization of healthcare resources and acts as a leader to achieve best practices of case management. To ensure the patients admitted to Palos Hospitalmeet the requirements for acute care and to assist in the transition of patients to the appropriate level of care.
Qualifications
Current RN licensein the State of Illinois. A BSN is preferred. Three years of recent clinical experience required. Completion of Case Management Certification within two years in position. It is also expected that once certification is obtained, the individual will be responsible for maintaining the certification while working as a case manager. This position may cross train in the Emergency Department.
