Job Description

One Health is looking for a Community Health Advocate (CHA) to support the resource needs of our most vulnerable patients. This position will be based at our Sheridan clinic and will be helping patients navigate social systems and barriers to care, such as insurance enrollments, referrals for basic needs, transportation, etc. You will be working with patients as part of the care team and developing and maintaining positive relationships with partners who provide support services.

The successful candidate is someone who:

• Loves interacting with people from a variety of backgrounds, professions and cultures
• Communicates well on the phone, in person, and in writing
• Has an open, calm, and professional demeanor (you how to put other people at ease!)
• Is really good at spotting issues, evaluating situations, and problem-solving within a team environment
• Loves making a positive difference in the lives of the people in our community

Required education/licenses/experience includes:

• High school diploma or equivalent (Bachelor’s degree or Associate’s level counselor, mental health aide, behavioral health aide, medical assistant, LPN, or nursing assistant, preferred)
• Valid Driver License
• BLS Certification upon hire
• Certified Applications Counselor Certificate within the first 12 months of employment
• Community Health Worker Certificate within the first 12 months of employment
