Clinical Policies and Procedures
Establish policies and procedures in accord with professional nursing standards of practice, standard procedures, and established State and Federal regulations for assisted living facilities
Implement policies and procedures through staff education, surveillance, mentoring, ongoing in-service education, and disciplinary actions as needed
Memory Care Programming
Continuously upgrade staff training and care protocols for residents in the memory care unit
Supervise the quality of care, safety, and life for residents in the dedicated dementia care unit
Staffing
Propose staffing ratios based on care management minutes for the current level of resident acuity. Specify minimum staffing for evening, night, and weekend shifts and the memory care unit.
Collaborate with HR manager to specify staffing needs, source, recruit, interview prospective resident care staff
Consistent with the approved staffing plan, make hiring decision on CNAs, LPNs and RNs in collaboration with Administrator and/or Mission Leader
Orientation
Establish reporting relationships for each resident care staff member and an up-to-date job description
Scheduling
Approved earned time off for resident care staff
Supervision
Provide daily written assignments (ElderMark generated) for nurses and resident care assistants based on residents’ Individual Service Plans
Verify nursing task completion through daily audits of ElderMark documentation
Monitor compassionate respectful attitudes and behaviors toward residents, family members, and other staff members
Make rounds throughout the day and periodically on all shifts
Conduct “stay” interviews with new hires at set intervals (e.g., 2 weeks, 1 month, 3 months)
Department Management
Serve on the leadership team of the ministry
Oversee all clinical services and ensure accountability and care delivery is provided to our organizational standards and the satisfaction of our residents and their loved ones
Implement and monitor compliance with resident care policies and procedures
Exercise skills of problem-solving and balanced decision-making on behalf of residents and within available resources
Develop a culture of outstanding professionalism and customer service
Promote empowerment and teamwork of all nursing staff in their delivery of care
Facilitate the participation of resident care staff in mission integration activities and resident life events
Meet with a resident care staff member who is challenged to be on time, work a full shift, or frequently absent. If otherwise a good performer, seek ways to accommodate the needs of both ministry and staff member
Financial Management
Collaborate with the Finance Director to develop the annual operating and capital budgets for the Resident Care Department
Manage the department within the approved operating and capital budgets
Verify time clock entries of resident care staff and submit payroll records
Offer professional advice on major capital projects (i.e., Higher Acuity Unit)
Information Systems
Regularly audit the accuracy and completeness of data entry on a resident’s condition and care and services received
Request customization of ElderMark and other software in order to increase the effectiveness and efficiency of its use
Direct Care
Take turns serving as the on-call nurse on evenings, night, and weekends
Mediation Administration
Implement a system to monitor medical administration, its timeliness, accuracy, adverse event monitoring (through ElderMark’s MAR)
Implement recommendations of the consulting pharmacist to improve medication administration and relay any suggestions to PCPs
Infection Prevention & Control
Stay current on infection prevention and control practices
Maintain daily focus on prevention and control with onsite surveillance and mentoring
Manage any outbreaks of COVID-19 or other infectious disease including preparations of sufficient PPE, refresher training, required reporting, and coordination with public health agencies
Family Relations
Manage complaint resolution and service recovery with residents and family members
Risk Management
Participate in the investigation of corporate responsibility complaints, their resolution, and reporting
Qualifications:
Bachelors’ degree/graduate in nursing, allied health related discipline or related field preferred, or equivalent experience
3+ years of geriatric experience
3+ years of proven experience in providing and supervising health care or human service professionals
Current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Wisconsin
Current CPR certification
Advanced knowledge in Adult Nursing Practice utilizing the Nursing Process standard
Excellent clinical knowledge and skills
Demonstrated ability to analyze an issue, identify options, and implement solutions
Knowledge of clinical, social, psychological and spiritual needs of residents
Ability to work cooperatively with people in and outside of Resident Care Services department
Ability to communicate (verbally and in writing); effectively interact with residents, their family members, supervisors and other staff members; provide work direction, and guidance to others
Knowledge of certifying and regulating agencies rules and regulations of CBRF facilities
Computer and software knowledge to effectively perform the tasks required
Detail oriented; ability to multi-task, set priorities, work under pressure, and meet deadlines
Typical Schedule: Incumbent would typically work Monday – Friday 8:00 – 5:00. However, weekend and after hours work could be required to support employees and mission of the Villa St. Francis.
Equipment Used: Office equipment: personal computers, printers, phone systems, fax machine, photocopier Resident Care Equipment: Medication/treatment cart, blood pressure gauge, stethoscope, sterile/non-sterile treatment supplies, adaptive devices, thermometers, wheelchairs, walkers, oxygen, and nebulizer equipment
Environmental Conditions:
Exposure to blood, body tissues, and other potentially infectious fluids—occasionally
Exposure to hazardous waste materials—occasionally
Exposure to loud or unpleasant voices and bodily injury due to unpredictable behavior of residents—rarely
Exposure to unpleasant odors related to incontinence of residents—rarely
Investigate an alleged resident abuse or neglect, document findings, and coordinate with Administrator on appropriate actions including required reporting to regulatory agencies
Investigate accidents resulting in injury on the premises, rendering first aid as needed. Document findings and recommendations to mitigate risks
Quality Assessment & Performance Improvement
Track, trend and report clinical quality data to identify risks
Facilitate the selection of audit studies, root cause analysis, and recommendations
Develop and implement performance improvement plans that measurably improve resident care and safety (e.g., fall prevention)
Communicate on a regular basis with leadership team members regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.
Regulatory Compliance
Maintain regulatory compliance with State, Federal, and local standards regarding quality resident care and services
Accompany and assist State surveyors conducting an annual or complaint visit
Collaborate with leadership team to conduct periodic mock surveys for regulatory compliance
